Advanced Nurse Practitioner - Learning Disabilities

Overview

An exciting opportunity has arisen within the Warrington and Halton Learning Disability Community Team for an Advanced Practitioner to be a key senior member of the Intensive Support Function of the team.

Key Responsibilities

• Provide expert clinical practice to clients posing significant challenges.
• Lead role in coordinating access to health care services.
• Conduct complex assessments and risk management plans.
• Training for a wide range of internal and external personnel.
• Ensure safe management of medicines and adherence to professional standards.

Qualifications and Skills

• Experience in managing complex cases.
• Autonomous practitioner with excellent communication skills.
• Knowledge of Mental Health Services policies.

Salary and Benefits

Salary range is £46,148 - £52,809 per annum.
