An exciting opportunity has arisen for District Nurses to join NELFT North East London Foundation Trust's Integrated Teams in Havering.

The successful candidate will be a Registered Nurse (RN12-NMC) responsible for patients allocated to them and will assess, manage, plan, and deliver care, including being professionally and legally accountable.
They will support their peers and team leader while working on their own initiative, supervising pre-registration students and committing to collaborative working by actively supporting and liaising with other health and social professionals and agencies.
Main duties of the job
The post holders will lead and manage a Community Nursing Team, aligned with GP practices and responsible for the management of a caseload, ensuring high standards of nursing care to patients and their families in the community. They will work as part of the integrated primary/community health care team, delivering individualised care and liaising with GPs and other professionals.

This post attracts a High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and maximum payment of £5,735 per annum).

Qualification
- District nurse qualification, certificate, DiP, Degree
- Current NMC Registration Registered Nurse (RN)
- Nurse/Prescriber/Non-Medical Prescriber
- Evidence of professional updating
Experience

- Providing complex packages of care
Skills
- Basic awareness of IT and IT skills
