This is an exciting opportunity for an experienced Health Practitioner Assistant/Health Care Assistant to join the Integrated Community Reablement Assessment Service (ICRAS) in Liverpool. The role is crucial in providing care and support for patients who require assistance to remain at home, rather than being admitted to hospital. Alternatively, it supports those who have been hospitalized, ensuring their assessments for care occur in the comfort of their home.
Key Responsibilities
- Work collaboratively as a member of the multi-disciplinary team.
- Perform patient interventions within the scope of NVQ level 3 skills, as delegated by the team leader or other qualified clinical staff.
- Provide advice on health issues, including health promotion and lifestyle advice.
- Organize and plan your work schedule after receiving delegation from the team leader or Senior Nurse Practitioner.
- Conduct basic patient observations.
- Adhere to clinical guidelines, policies, and infection control procedures.
- Support patients and families across various situations.
- Document information accurately while maintaining confidentiality.
- Demonstrate your role to new staff and students.
- Utilize safe moving and handling techniques in patient care, including equipment usage.

Qualifications
- NVQ Level 3 in Health & Social Care or equivalent.
Experience & Knowledge
