Intensive Outreach Mental Health Practitioner

Overview

Due to retirement, Essex Partnership University NHS Foundation Trust currently have a vacancy for a Band 6 nurse, OT, or Social Worker to join the Intensive Outreach Team covering South East Essex. The post-holder will manage a small caseload of service-users who present with complex needs.

Job Responsibilities

• To assess, plan, deliver and evaluate care for service users referred to the Intensive Outreach Team.
• Manage a caseload as the named key worker/care coordinator.
• Work alongside service users in daily activities, budgeting skills, and social structures.
• Develop therapeutic relationships to provide necessary support.
• Participate in mental health promotion and education.
• Administer prescribed medication and monitor effects.
• Provide clinical supervision to students and other staff within the team.
