The Lower Limb Service Nurse Co-ordinator is an exciting role for a Registered Nurse (RN12-NMC) responsible for patients allocated to them. They will assess, manage, plan, and deliver care, while being professionally and legally accountable. The post holder will provide specific lower limb advice and support to patients and their caregivers, focusing on coordinating the lower limb care service within the Locality team.
Main duties of the job
The post holder, supported by the team lead, will lead and manage the lower limb service within the Locality team. Responsibilities include:
- Providing high standards of nursing care to patients and their families in the community.
- Liaising with GPs and other professionals to deliver individualized care.
- Working independently while providing leadership to the community nursing team.

Probationary Period: This post will be subject to a probationary period, with internal applicants exempt unless already partway through a probation.

High Cost Area Supplement: The role attracts a High Cost Area Supplement of 15% of the basic salary, with minimum and maximum payments specified.

Person Specification
Knowledge
- Awareness of NHS Plan, NSF, and clinical governance priorities.
- Extensive clinical skills including assessment and care management.
- Understanding of clinical governance and its application.
Qualifications
- Current NMC registration - Registered Nurse (RN).
- Post Graduate qualification in a relevant field.
Experience
- Extensive experience within adult services.
- Providing support and supervision to community team members.
