About the Role

East Suffolk Primary Care Network is looking to recruit a pharmacy care co-ordinator to work as part of the pharmacy team to improve quality and safety of care offered to patients of the primary care network. You will have regular contact with patients and support the delivery of the pharmacy service.  The role is offered on a part-time basis for up to 30 hours a week; we can be flexible about the number of hours and days over which they are worked.

 

The main duties of the role include:

  • To develop and facilitate a good working relationship with the practices within the PCN and other local providers 
    of healthcare.
  • To plan and organise own workload, including audit and project work, and training sessions for members of 
    the practice team.
  • To attend local, regional and national meetings of relevance, as agreed with the post holder’s line manager.
  • Key responsibilities include:
    • booking structured medication reviews
    • pharmacy service impact data reports
    • manage audit workflow
    • support high risk drug management
    • pharmacy tasks/clinics
    • monitor pharmacy shared email
    • care-home workflow

About the Candidate

The ideal applicant will have have the following skills, experience and qualifications:

  • GCSE grade A to C in English and maths, or equivalent;
  • Ability to collate and disseminate voluminous and sometimes complex information;
  • Ability to assess and work within an individual's level of health literacy and to support them to increase their understanding;
  • Proven record of excellent written and verbal communication skills;
  • Experience of working with healthcare professionals and/or previous experience in the NHS, social care or another relevant field;
  • Experience providing advice/signposting to patients;
  • Experience of coordinating and liaising with multiple stakeholders or individuals, to meet specified outcomes;
  • Experience of using technology and digital tools to support health and wellbeing.
