NELFT North East London Foundation Trust is looking for an experienced Mental Health Practitioner to join NELFT North East London Foundation Trust's Thurrock PCN for 12 months fixed term. As part of the NHS long-term plan, local areas are aligning community mental health services with primary care networks, creating new integrated models of care.
PCN's Objectives
- Co-ordinate at the heart of GP practices as an integrated member of the multi-disciplinary team.
- Provide consultation, advice, triage, and liaison in accordance with agreed protocols.
- Support children and young people with mild to moderate mental health issues through evidence-based interventions.
The Mental Health Senior Practitioner will be responsible for assessment and caseload management within the GP PCN, triaging cases, conducting full assessments, and managing the caseload of children and young people (CYP) with mental health needs.
Main Responsibilities
This role supports the development and delivery of the PCN service in Essex. Responsibilities include:
- Delivering evidence-based low-intensity interventions in GP services.
- Collaborating with GPs to support CYP and their families in self-managing mental health difficulties.
- Managing a clinical caseload and ensuring service targets are met.

This post attracts a High Cost Area Supplement of 5% of the basic salary, with minimum and maximum payments specified.
Job Responsibilities
The PCN CYP MH Practitioner will work with locality-based Clinical Leads and will be community-based, spending time in primary care settings. Responsibilities include:
- Assessing and managing patients using expert knowledge.
- Producing and completing individual care plans.
- Providing specialist clinical support and advice.
- Educating and involving family members in treatment.
- Managing personal workloads and adhering to departmental policies.

- Experience in community mental health settings.
