The successful applicants will be based in one of the integrated community mental health teams in Hammersmith and Fulham as part of the community and Recovery Mental Health Services service line. This role will involve:

  • Working within a fully integrated mental health team with a visible presence in the local Primary Care Network.
  • Implementing a tiered or stepped care model within the MINT team, focusing on primary care delivery for the majority of patients.
  • Providing enhanced support for patients with complex needs through allocated clinicians and the multidisciplinary team.
  • Engaging in consultation/liaison with GPs, primary care services, and other mental health teams.
  • Direct consultation with patients, especially around complex cases and crisis prevention.

Key working relationships will be with GPs, the Crisis Team, and the Early Intervention in Psychosis Service. This is a really exciting role as part of a fully integrated service, fulfilling expectations of the new community framework.

Responsibilities

  • Developing safety and care planning for service users in line with new NHS working practices.
  • Maintaining good working relationships across all mental health teams.
