Mersey Care NHS Foundation Trust's Southport & Fomby Urgent and Intermediate Care therapy team is seeking committed Band 6 Occupational Therapists to join Mersey Care NHS Foundation Trust's service, supporting patients through shared care within their localities. Mersey Care NHS Foundation Trust is looking for highly skilled, enthusiastic, and proactive therapists to join Mersey Care NHS Foundation Trust's multi-disciplinary team, which includes Physiotherapists, Occupational Therapists, Assistant Practitioners, Health Practitioner Assistants, Community Discharge Matrons, Nursing staff, and social workers.
Main Duties
- Manage a caseload of patients' co-morbidities as an autonomous practitioner.
- Provide leadership for junior staff through supervision and appraisal.
- Participate in planning, development, and evaluation of OT services.
- Take a holistic approach in assessing and treating service users.
- Work in partnership with other professionals to prevent hospital admissions.
- Contribute to the development of the Occupational Therapy profession within ICRAS.
Job Responsibilities
- Undertake comprehensive holistic assessments and devise treatment plans.
- Engage patients to gain consent through their therapy journey.
- Prioritize Occupational Therapy referrals based on need and risk.
- Plan and implement patient-centered interventions.
- Monitor and evaluate treatment effectiveness.
- Provide clinical advice and leadership within your team.
- Liaise with family, carers, GPs, and other professionals.
- Develop risk management plans and monitor service user needs.

Qualifications
Essential
- Relevant professional OT qualification
- Current HPC registration
Desirable
- Fieldwork Educators certificate
- Post graduate accredited study (e.g., PSI, CBT, MSc in relevant health studies)
Knowledge/Experience
