The Extra Care Housing Team is a small county-wide team that works with adults 18+ living within Extra Care housing, providing accommodation with onsite care to customers who require support. This model offers an alternative to residential care.
Role Overview
As a Social Worker, you will be an integral part of a dynamic operational team within Adult Services. Your contributions will include:
- Day-to-day management of Extra Care Schemes
- Working with stakeholders to ensure high-quality service delivery
- Undertaking strengths-based assessments applying relevant legislation
- Developing and implementing support plans aimed at improving wellbeing
Key Responsibilities
Your responsibilities will include:
- Managing a caseload and collaborating with individuals and their families
- Conducting assessments and identifying eligible needs
- Utilizing community resources to meet individual needs
- Receiving professional supervision and support to enhance your skills and confidence

Qualifications and Experience
This position requires:
- A degree in social work
- Registration with Social Work England
- Experience in working with vulnerable populations
